29 CFR 1910.1000(c): The did not ensure that any substance listed in Table Z-3, in any 8-hour work shift of a 40-hour work week, did not exceed the 8-hour time weighted average limit given for that substance in the table: (a) Halliburton Energy Services, Inc. @ Weicheil Wellhead 27N-11HZ, Hudson, CO 80642: On and before 11/19/12, the employer did not ensure that employees exposed to respirable silica were below the permissible exposure limit (PEL). An employee, Mountain Mover operator, was sampled for 471 minutes. He was exposed to 48% silica. The calculated PEL for 48% silica is 0.2000 mg/m3. His calculated 8 hour time weighted average was 0.4015 mg/m3. His calculated severity was 2.0 times the PEL. (b) Halliburton Energy Services, Inc. @ Weicheil Wellhead 27N-11HZ, Hudson, CO 80642: On and before 11/19/12, the employer did not ensure that employees exposed to respirable silica were below the permissible exposure limit (PEL). An employee, Mountain Mover operator, was sampled for 461 minutes. He was exposed to 47% silica. The calculated PEL for 47% silica is 0.2041 mg/m3. His calculated 8 hour time weighted average was 0.5788 mg/m3. His calculated severity was 2.8 times the PEL. 29 CFR 1910.134(g)(1)(iii): Employee(s) did not perform a user seal check each time they put on a tight-fitting respirator using the procedure in Appendix B-1 of the standard or procedures recommended by the respirator manufacturer that the employer demonstrated were as effective as those in Appendix B-1: (a) Halliburton Energy Services, Inc. @ Weicheil Wellhead 27N-11HZ, Hudson, CO 80642: On and before 11/19/12, the employer did not ensure that employees performed user seal checks. Mountain Mover operators would not perform seal checked following the removal of their 3M, 6600 Series, half-faced, air purifying respirators from their face so they could communicate with each other in the noisy work environment. This condition exposed employees to respirable silica.
