29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 235 Great Pond Drive, Windsor, CT 06095: On or about 11/05/2012 an employee was attempting to clear a paper that was jammed in a mail sorting machine without utilizing the established procedure resulting in hand crushing injuries.
