29 CFR 1910.23(e)(5)(iv) The mounting of handrails was not done such that the completed structure is capable of withstanding a load of at least 200 pounds applied in any direction at any point on the rail. On October 23, 2012, at Elwood A#12, Rig 1661 a handrail near a stairway was not structurally capable of withstanding a load of at least 200 pounds applied in any direction. The handrail was not installed using the both securing pins.

29 CFR 1910.305(g)(2)(iii) Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that would prevent pull from being directly transmitted to joints or terminal screws. On October 23, 2012, at Elmwood A #12, Rig 1661, several flexible cords for motor systems were observed to not have the appropriate strain relief so as to prevent pulls from being directly transmitted to joints, exposing electrical conductors to damage.
