29 CFR 1910.147(c)(7)(i)(C):     The employer failed to ensure that all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.    Job site located at 2415 Ashland Ave, Beatrice, NE:  The employer failed to ensure that any employee affected by lockout/tag out operations had received the level of training necessary to ensure a safe level of understanding of the lockout/tag out program.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Job site located at 2415 Ashland Ave, Beatrice, NE:  Three pieces of equipment in the area were not equipped with machine guarding to prevent the employees from contacting rotating parts or other injurious movements:    a)  Bridgeport Milling Machine, Serial Number 030761    b)  Wilton Strand Machine Systems Drill Press    c)  South Bend Lathe Works metal lathe (manufacturer unknown)    Abatement certification is required for this violation. 29 CFR 1910.1200(h)(3)(iii):     Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:    Job site located at 2415 Ashland Avenue, Beatrice, NE:  The employer failed to provide training as prescribed in the paragraph above in that the employees had not been trained on the hazards of the chemicals that they commonly use including but not limited to Carbon Steel solid wire electrodes and rods (welding rod byproducts), Lead Acid Battery (sulfuric acid and water mix), and welding flux.      Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
