29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when: (B) The operator has been involved in an accident or near-miss incident; On or about February 3, 2026, at an establishment located at 1055 West Ryan Street, Brillion, Wisconsin; in the Orange Cell by the 503 North Press, the forklift operator was not retrained and evaluated following a near miss involving a scrap bin, work table, and antifatigue mats exposing employees to struck by hazards.

29 CFR 1910.178(g)(2): Facilities shall be provided for flushing and neutralizing spilled electrolyte, for fire protection, for protecting charging apparatus from damage by trucks, and for adequate ventilation for dispersal of fumes from gassing batteries. On or about February 11, 2026, at an establishment located at 1055 West Ryan Street, Brillion, Wisconsin; in Plant 2 by Door 311, the electric forklift charging station did not have an eyewash station readily available exposing employees to eye injuries.
