29 CFR 1926.1052(c)(1)(ii):     Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a)  The employer did not ensure that stairways having four or more risers were equipped with a stairrail system along each unprotected side or edge.  This violation occurred when employees used a wood staircase in Unit seven (7) to access upper levels that had one unprotected side without a stairrail system.     b)  The employer did not ensure that stairways having four or more risers were equipped with a stairrail system along each unprotected side or edge.  This violation occurred when employees used a wood staircase in Unit five (5) to access upper levels that had one unprotected side without a stairrail system.     c)  The employer did not ensure that stairways having four or more risers were equipped with a stairrail system along each unprotected side or edge.  This violation occurred when employees used a stairwell between the third (3rd) and fourth (4th) floor without a starrail system on the unprotected sides of two levels of stairs containing six (6) risers.

This violaiton occurred when two spliced flexible cord sets (an fourteen (14) gauge and a twelve (12) gauge) were used without a ground pin in the plug end of the twelve (12) gauge cord.

29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a)  The employer did not ensure that flexible cords were connected to devices and fittings so that strain relief was provided to prevent pull from being directly transmitted to joints or terminal screws.  This violation occurred when a fan was used with a flexible cord that was damaged at the plug joint from strain pulling away the outer insulation.
