29 CFR 1926.453(a)(2):     Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2.    a) Employees operating an Elliott truck mounted boom lift were exposed to injuries from an improperly repaired or modified platform floor.  A hole in the expanded steel floor was covered with a piece of plywood.
