29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees    a. 1 Brady Road, Lake Hopatcong, NJ -     Employees observed working in a trench that was more than 4 feet in depth without a a safe means of egress.  The employer did not ensure that ladders and other safe means of exit from the trench were positioned as to be no more than 25 feet away from any worker in the trench.     Violation observed on or about November 12, 2012

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i)    a. 1 Brady Road, Lake Hopatcong, NJ     Employees performing trenching activities by hand were not protected from cave-ins by an adequate protective system.  Employer did not provide sloping or benching, trench boxes, or shoring to protect employees from cave-in hazards associated with the trenching activities.    Violation observed on or about November 12, 2012
