29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) First Floor Southeast Side - Employees were observed stepping over a floor hole that measured 25 and a half inches wide and exposed to a 10 feet, fall hazard.     b) Third Floor West Side - A laborer was hosing down a concrete jack hammer operation to control dust while standing near a floor hole that measured 33 inches wide, 77 inches long and exposed to a 34 feet, 10 inch fall hazard.
