Governor's Glen Assisted Living- On or about November 13, 2012, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012. (a) On or about October 3, 2012, a Resident Assistant received a back injury while lifting a resident. The employee was placed on restricted duty. On or about October 16, 2012, the Resident Assistant that was on restricted duty was reinjured while assisting a resident who had fainted. The employee was placed on disability on November 3, 2012.
