29 CFR 1904.39(a): An oral report of a work-related incident resulting in the death of any employee was not made to the nearest Area Office of the Occupational Safety and Health Administration within 8 hours after the occurrence: Location: 1906 Maritime Blvd. Tampa, FL a) on November 12, 2012 at 11:20 pm, an employee was found on the floor unresponsive and was taken to a nearby hospital where he was pronounced dead on November 13, 2012 at 12:09 am. The fatality had not been reported to OSHA until November 14, 2012 at 8:00 am.
