29 CFR 1926.453(b)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    North Jobsite - On or about November 14, 2012 employees were exposed to a fall hazard of 12 feet while working from a Genie S40 aerial lift without wearing a body belt or body harness and lanyard attached to boom or basket.
