29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a. At the jobsite, employees were not informed on the procedures to apply, dispense or handle chemicals such as but not limited to cleaner, sealant and bonding adhesive, thereby being exposed to contact hazards.

29 CFR 1926.404(b)(1)(iii)(C):     Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.    a. At the jobsite, a yellow 50' extension cord with a missing ground prong was not inspected prior to each days use, thereby exposing employees to a contact hazard.    b. At the jobsite, a heat gun with insulation damage to the cord was not inspected prior to each days use, thereby exposing employees to a contact hazard.    c. At the jobsite, a Dewalt screw gun with insulation damage to the cord was not inspected prior to each days use, thereby exposing employees to a contact hazard.    d.   At the jobsite, an orange 50' extension cord with exposed interior conductor insulation was not inspected prior to each days use, thereby exposing employees to a contact hazard.

29 CFR 1926.405(g)(2)(iv):     Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    a. At the jobsite, employees were using extension cords with exposed interior wire insulation at the plug due to the strain relief being pulled away, thereby exposing employees to a contact hazard.

29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    a. On the second story of the building, an employee was working on the edge of the roof approximately twenty four feet above ground level without having a fall protection system in place, thereby being exposed to a fall hazard.    b. On the first floor roof overhang, an employee was working on the roof approximately thirteen feet above ground level without having a fall protection system in place, thereby being exposed to a fall hazard.    Modern Exteriors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1920.501(b)(10), which was contained in OSHA inspection number 311837348, citation number 1, item number 2, and was affirmed as a final order on September 29, 2008, with respect to a workplace located at 1094 McKinley Avenue, Columbus OH 43222.  Modern Exteriors was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1920.501(b)(10), which was contained in OSHA inspection number 313153645, citation number 2, item number 1, and was affirmed as a final order on February 2, 2010, with respect to a workplace located at 22 North 23rd Street, Newark OH 43055.
