At Boise Packaging and News Print, L.L.C., Deridder, LA, the employer did not record the following workplace injurie correctly on the OSHA 300 Log for calendar year 2012    On or about August 06, 2012, 0801 was using a long crowbar trying to remove debris from a furnace and received a blow to the right side of the head which resulted in loss of consciousness. The employee received sutures and a CT scan of the brain and was admitted overnight to the hospital and released the next day.
