29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;    (a) Island Concrete - The employer did not perform a hazard assessment of the work area to determine the type of personal protective equipment that is required while using products such as but not limited to portland cement that contains hexavalent chromium, silica and Daracem 55, on or about November 30, 2012.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:    (a) Island Concrete - The employer did not provide employees with effective information and training for chemicals such as but not limited to concrete, portlant cement and doracem 55, on or about November 30, 2012.  ABATEMENT NOTE:    1. Employees shall be informed of the following:  2. The requirements of this section;  3. Any operations where hazardous chemicals are present;  4. The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals, and Material Safety Data Sheets.  Employee training shall include at least:  1. Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area;  2. The physical and health hazards of the chemicals in the work area;  3. The measures employees can take to protect themselves such as specific procedures and personal protective equipment to be used;  4. The details of the employer's Hazard Communication Program including an explanation of labeling system, Material Safety Data Sheets, and how employees can obtain and use the appropriate hazard information.
