29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about November 15, 2012, at the Catawba Shores Condominiums in Port Clinton, OH: the employer did not assure employees were protected from fall hazards while applying roofing materials to a roof system. The employees were exposed to approximately an 18 foot fall to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about November 14, 2012, at the Catawba Shores Condominiums in Port Clinton, OH: the employer did not assure that employees were provided with safe access to the upper landing surface of a two story structure by extending the ladder side rails at least three feet above the surface to be accessed. Employees were exposed to an 18 foot fall hazard.
