29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.      a) Lake St., Lake Charles,  LA; employees were exposed to falling objects from heights up to approximately 18 feet while working from a scaffold that did not have toe boards or other devices used to prevent falling debris. Employees were applying brick to the exterior of a building. Potential falling objects from overhead included, but may not be limited to, bricks, hand tools, and five gallon buckets.

29 CFR 1926.451(h)(2)(ii): A toeboard shall be erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below, except on float (ship) scaffolds where an edging of 3/4 x 1 1/2 inch (2 x 4 cm) wood or equivalent may be used in lieu of toeboards;      a) Lake St., Lake Charles, LA; the employer failed to ensure toeboards were installed on a 13 foot high scaffold platform that held bricks and other material. Employees working underneath were exposed to a struck-by hazard.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.      a) Lake St., Lake Charles, LA; the employer failed to ensure a scaffold used for overhand bricklaying was equipped with a ladder for access and egress. The work platform was approximately 13 feet above the ground.
