29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas: On or about November 7, 2012 scrap 2x4 inch lumber had nails sticking up into the air created an impalement hazard at the construction site.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about November 7, 2012 the SJTW cord which energized a skillsaw was not grounded. The saw was located behind the apartment complex under construction.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about November 7, 2012 the male plug that connected the SJTW cord to the temporary power pole did not have a strain relief device, causing an electrical hazard. A carpenter was using the cord to energize his skillsaw at the construction site.
