On or about November 15, 2012, during tree trimming activities, the employer did not assess the worksite for hazards. The employer working in the aerial bucket truck and two employees working on the ground, under the employer and the bucket truck, did not have head protection. All three employees were not utilizing any eye or face protection while performing cutting and wood chipping activities. The proper PPE was not at the worksite.
