29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:    Job located at 1120 E. 10th St. Minneapolis, KS 67467 - Employer had failed to develop a written respiratory program that describes the protection procedures for employees that are required to wear a half face respirator when preparing a mix of Nitric Acid and Chrome additive.  Employees are exposed to Nitric Acid fumes.

The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.    Job located at 1120 E. 10th St. Minneapolis, KS - Employer had failed to provide a medical evaluation to employees that are required to wear a half face respitaror when preparing a solution of Nitric Acid and Chromate additive.  Employees are exposed to Nitric Acid Fumes.

Jobsite located at 1120 E. 10th St. Minneapolis, KS - Employer had failed to provide a fit test to employees that are required to wear a half face respirator when preparing a solution of Nitric Acid and Chrome flakes.  Employees are exposed to Nitric Acid Fumes.

29 CFR 1910.151(c):  Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    Jobsite located at 1120 E. 10th St. Minneapolis, KS - In the rack line and barrel line area, an eye wash facility capable of supplying continuous flow for 15 minutes was not provided for flushing of the eyes.  Corrosive chemicals such as Nitric Acid were used in the area, exposing employees to chemical burns to the eyes.

29 CFR 1910.1200(e)(1):  Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    Jobsite located at 1120 E. 10th Street, Minneapolis, KS - Employer had failed to develop a written hazard communication program that describes the handling of hazardous substances. Employees are exposed to chemical substances such as but not limited to hexavalent chromium, Nitric Acid, EC-12 dye and EA-96 Chromate, EA-97 and Chromate additive while conducting metal plating activities.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.    Jobsite located at 1120 E. 10th St. Minneapolis, KS 67467 - Employer had not provided formal training to employees on hazard communication.  Employees are exposed to skin and eye burns from acid and derivatives of chromates.
