On or about November 15, 2012, during roofing activities, the employer allowed a sub-contractor's employees to work on a roof that was approximately 11.4 feet high with a 5/12 pitch on the house and 8 feet high with a 4/12 pitch on the garage and did not ensure that the employees were protected from falls
