29 CFR 1910.303(b)(7)(i):  Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:    (a)  On or about November 15, 2012, and at times prior, employees were exposed hazards of live electrical components in the following locations because they were not effectively closed:    1)  The electrical box mounted on the north wall which supplied power to an RF Generator Rack had a hole in the door.  2)  Two plug boxes hanging from the ceiling had the face plate disconnected from the box and the live wires were out of the boxes.  3)  A plug on the south wall in the lab area, labeled "Unit 1-CT.13," was missing a face plate.  4)  The circuit breaker box mounted on the north wall of the building stood open.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
