29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers: (a) Penthouse Area - A twelve inch circular opening was cut through the floor area for the installation of a three inch steam pipe. The opening was left uncovered while employees were working at other areas in the Penthouse, tools, pipes and other items could fall through the opening striking employees on the floor below, on or about October 12, 2012.
