29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a)  On or about November 15, 2012, and at times prior, employees were exposed to fall hazards from the second floor of the warehouse when they walked outside the railings to retrieve boxes of product stored in areas located outside of the railings.  The storage areas outside the railings were approximately 10 feet above the adjacent ground level and were not guarded to protect against falls.      b)  On or about November 15, 2012, and at times prior, employees were exposed to falls of approximately 10 feet while they worked near the conveyor belt loading area on the second floor of the warehouse that had a horizontal opening of approximately 37 inches that was not protected by standard guardrails.      c)  On or about November 15, 2012, and at times prior, employees were exposed to falls of approximately 10 feet while they worked near the forklift loading area on the second floor of the warehouse that had a horizontal opening of approximately 83 inches that was not protected by standard guardrails.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:                                                                 a)  On or about November 15, 2012, and at times prior, employees were exposed to electrical shock and fire hazards while they used a power cord in the warehouse desk area that had strained and damaged insulation at the joint where the cord entered the plug. The cord was attached to a powerstrip which, among other things, powered a desk fan.
