29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about November 15, 2012, on the roof, the employer did not ensure that each employee working and walking on wood planks and floor joist within 2-feet of leading edge, with unprotected sides and edges of the roof, were protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems; exposing employees to the hazards of falls from an elevation of approximately 7 - 10 feet to lower levels below.

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about November 15, 2012, the employer did not ensure that an employee working from and A-frame ladder was properly using the ladder for the purpose in which it was designed. The employee was wood framing the interior walls of a two-story residential structure, while straddled over and standing on the second, top rung of an A-frame ladder . This condition exposed the employee to the hazard of stumbling and falling from the ladder.
