29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a) Baily Oxides located on 6191 County Road 10 in Delta, OH: On or about November 9, 2012, the employer did not document the completion of the measures required for a safe permit space entry operation by preparing an entry permit. A permit was not completed prior to employees entering into the absorber tower which has the potential to contain a hazardous atmosphere as a result of reclaiming acid used in the steel pickling process. Employees were in the absorber for approximately 4-hours removing packing material.
