29 CFR 1926.416(a)(1):     No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.    (a) The employees installing a new lighting system on a commercial building did not ensure that the electric power was deenergized. The employees were exposed an electrical shock hazard.

29 CFR 1926.417(a):     Controls. Controls that are to be deactivated during the course of work on energized or deenergized equipment or circuits shall be tagged.    (a) The employer did not ensure that controls related to the flow of electrical energy were tagged. The employees were installing a new lighting system on a commercial building and did not ensure that the energized or deenergized equipment was tagged. The employees were exposed to an electrical shock hazard.

29 CFR 1926.453(b)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    (a) The employer did not ensure that an employee was using either a fall arrest or fall restraint system when conducting work on a commercial building. The employee was conducting electrical work and was exposed to a fall hazard.

29 CFR 1926.501(b)(1):     "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) An employee was replacing and installing an exterior lighting system on a commercial building and was exposed to a fall hazard.
