29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and a guardrail system.    On November 16, 2012 at a worksite located at 704 E. Veterans pkwy. Yorkville IL, an employee was performing roofing work and was not using any means of fall protection. The employee was exposed to a fall hazard of approximately 18.8 feet.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903(d).

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On November 16, 2012 at a worksite located at 704 E. Veterans pkwy. Yorkville IL, an employee was accessing the roof by an extension ladder that was not three feet above the landing. The employee was exposed to fall hazard while accessing the roof.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903(c).
