29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Jobsite: on or about 11/16/2012, laborer was working on the lower level of the residents below to other employees using hammers, a pneumatic nail gun, bundles of shingles and roles of roof membrane on the upper level and exposed to potential head injuries from those items falling approximately 7 feet.

This exposed 3 employees to fall hazards of approximately 20 feet and one employee to a 10 foot fall hazard.
