29 CFR 1926.451(f)(14):     Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:    On or about November 15, 2012, on a mobile scaffold, where the employer did not ensure an employee understood  how to safely use a mobile scaffold. An employee was standing on a CMU block on top of the platform of a mobile scaffold to increase his working height while installing an interior steel top plate  This practice created a condition which exposed the employee to tipping over and falling to the unfinished surface below.

29 CFR 1926.452(w)(2):     Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    On or about November 15, 2012, on the work platform, where the employer did not ensure employees were using a mobile scaffold with all casters and wheels locked and capable of being locked, to prevent movement of the scaffold. An employee was installing an interior steel top plate while standing on a Grabber mobile scaffold and a cement masonry unit (CMU) block. During the installation process, the employee did not have the scaffold casters and wheels locked to prevent movement. This practice created conditions which exposed the employee to stumbling and falling from the scaffold to the unfinished surface below.

29 CFR 1926.452(w)(9):     Caster stems and wheel stems shall be pinned or otherwise secured in scaffold legs or adjustment screws.    On or about November 15, 2012, on the work platform, where the employer did not ensure employees were using a mobile scaffold with all casters and wheels stems pinned or secured in scaffold legs. An employee was installing an interior steel top plate while standing on a Grabber mobile scaffold and a cement masonry unit (CMU) block. During the installation process, the employee did not have the scaffold casters and wheel stems pinned or secured to the legs of the scaffold to prevent tipping of the scaffold during movement. This practice created conditions which exposed the employee to tippping and falling from the scaffold to the unfinished surface below.

29 CFR 1926.452(w)(3):     Manual force used to move the scaffold shall be applied as close to the base as practicable, but not more than 5 feet (1.5 m) above the supporting surface.    On or about November 15, 2012, on a mobile scaffold, the employer did not ensure an employee understood how to relocate a mobile scaffold. While standing on a CMU block on top of the platform of a mobile scaffold at an approximate elevation of 6'-9", an employee was rocking back and forth in order to manually force forward movement of the scaffold. This practice created a condition which exposed the employee to tipping over and falling to the unfinished surface below.
