29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Hooters Roof, 8909 E. Indian Bend Road, Scottsdale, AZ (Salt River Pima-Maricopa Indian Reservation): On or about November 14, 2012, employees were at the edge of the roof without fall protection, approximately 18 feet above the ground, when they installed a Banner along the top edge of the building.
