29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy in accordance with the requirements noted in the 29 CFR 1910.147(c)(4)(ii)(A) through 29 CFR 1910.147(c)(4)(ii)(D):      A.)  White-Brook, Inc. - On or about 11/14/12, employees performed maintenance and servicing on equipment including but not limited to gas furnaces and a Dake 25 Ton Hydraulic Press.  The lockout/tagout procedures did not clearly identify all of the specific steps to be followed by employees to isolate and disable the machinery in order to safely de-energize the equipment and control the hazardous energy. The procedures did not include specific sequential procedural steps to detail an effective lockout, steps to verify deenergization, notification of authorized employees before performing lockout and before releasing the lockout, physical location of disconnects and other energy isolation points, instructions on how to drain/relieve stored energy, energy sources, and magnitudes of the energy sources.      ABATEMENT CERTIFICATION REQUIRED:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement and state that employees and their representatives were informed of this abatement.

29 CFR 1910.215(b)(9):     Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:        A.)  White-Brook Inc., upper level - On or about 11/14/12, a skil electric bench grinder (Serial #18G00D0440) was not equipped with a tongue guard.    ABATEMENT CERTIFICATION REQUIRED:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement and state that employees and their representatives were informed of this abatement.

White Brook, Inc. - The entries in Column F on the OSHA 300 Log for Calendar Year 2012 were not descriptive in that the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  These entries occurred with cases that included but were not limited to Case #01.
