29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On top of the drive through on the north exterior elevation of Harbour Bank under construction located at 75140 South U.S. 1 in Port Saint Lucie, Florida:  An employee performing carpentry work on a walking/working surface approximately 14 ft.  8 inches above a lower level with unprotected edges was not protected from falling to that lower level, on or about November 14, 2012.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On site of Harbour Bank under construction located at 75140 South U.S. 1 in Port Saint Lucie, Florida:  The employer did not provide a training program for an employee performing carpentry work on a walking/working surface that would enable him to recognize the hazards associated with falling, on or about November 14, 2012.    PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On the north exterior elevation of Harbour Bank under construction located at 75140 South U.S. 1 in Port Saint Lucie, Florida:  An employee gaining access to and from a walking/working surface performing carpentry work approximately 14 ft. 8 inches above a lower level using an aluminum extension ladder did not have the side rails of that ladder extended at least 3 ft. past the upper landing surface to which he was gaining access to, on or about November 14, 2012.
