(a)           Bridge Over Mahantango Creek - An employee was using a double insulated Milwaukee hand held circular saw (serial number 983DD08320306) connected to an extension cord. The extension cord was plugged into an outlet at the work trailer.  There was no GFCI protection on the outlet, on or about October 18, 2012.
