29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. (a) Bridge Over Mahantango Creek - An employee was using a double insulated Milwaukee hand held circular saw (serial number 983DD08320306) connected to an extension cord. The extension cord was plugged into an outlet at the work trailer. There was no GFCI protection on the outlet, on or about October 18, 2012.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Bridge Over Mahantango Creek at SR 2022 - The extension cord that was supplying power to the double insulated Milwaukee heavy duty hammer drill was plugged into the temporary power outlet. The extension cord was missing the ground pin at the male end, on or about October 18, 2012.
