29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.        (a) Jobsite:  On or about November 14, 2012, an employee was working from a pump jack scaffold and he was not protected from falling at either end of the pic.  The fall hazard to the employee was approxiamtely 20 feet.

29 CFR 1926.1053(b)(3):     Ladders were used for a purpose other than what they were designed:        (a)  Jobsite:  On or about November 14, 2012, the company was using an aluminum extension ladder as a work platform on a pump jack scaffold.  The company was storing a box of vinyl siding on this horizontal ladder.
