29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.    On or about November 15, 2012, employees were exposed to approximately 11.4-foot fall hazards when the scaffold was not assembled under the supervision of a competent person who would have recognized deficiencies such as, but not limited to missing fall protection and poles that were not braced at the bottom.

On or about November 15, 2012, employees were exposed to approximately 11.4-foot fall hazards when the employees were working on a pump jack scaffold installing siding on the back of building 1503 without fall protection such as, but not limited to, nets, guardrails, or a personal fall protection system (harness, lanyard, and anchorage).
