This violation occurred on or about November 16, 2012 when employees on the fifth (5th) buck of a fabricated frame scaffold were required to walk between two sets of scaffolds on a walkway that was not protected by a guardrail system on any side of the walkway.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. a) The employer did not ensure that employees on a scaffold were provided with additional protection from falling objects. This violation occurred on or about November 16, 2012 when employees working on the fifth (5th) buck of a fabricated frame scaffold were not protected from falling objects from the sixth (6th) buck by the installation of toeboards, screens, guardrail systems, debris nets, catch platforms or canopy structures.
