29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use. Location: Worksite A written respiratory protection program was not developed and implemented for employees wearing respirators during breaking of cathode ray tubes contianing lead. Such program shall include: 1) Procedures for selecting respirators 2) Medical evaluations for employees who wear tight-fitting elastomeric respirators 3) Procedures for cleaning, maintaining, disinfecting, storing, inspecting, repairing, and discarding respirators 4) Training of employees on the respiratory hazards to which they are exposed 5) Training in the proper use of respirators 6) Procedures for resgularly evaluating the effectiveness of the program
