29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 11/19/12 in light court A, inside Holley High School, 3800 N. Main St., Holley, NY: A Black & Decker Drillmotor with a mixing attachment was used to mix thin set mortar in a pail. The drill was connected to 120 volt-ac power via a series of two extension cords, the first of which was plugged into a temporary receptacle in room 1401. That first extension cord lacked a ground prong on its' plug. NO ABATEMENT CERTIFICATION REQUIRED
