Employee(s) exposed to a caught-in hazard. The type SN55, number, 0552079 made in Czechoslovakia.    b) On or about November 20, 2012, and times prior thereto, the big lathe was not guarded.  Employee(s) exposed to a caught-in hazard.  The lathe was made by American Tool Works Co., with a  General Dynamics Electric Boat coding number 734700.

The wheel  was  1 inch from the tongue guard.  Dayton bench grinder, model  #4Z911B, serial 9810.  Employee(s) exposed to a struck-by hazard.      b) ) On or about  November 20, 2012, the Dayton bench grinder , the adjustment on the right  wheel was greater than 1/4 inch.
