An employee was on the 12 inch wide "pik" of the scaffold hanging siding, and was exposed  to a fall of approx. 14 ft. to the lower roof below. The employee was not protected by a personal fall arrest system, nor was any other fall protection system being utilized.    NO ABATEMENT CERTIFICATION REQUIRED

An employee was on the 12 inch wide, 20 ft. long, aluminum "pik" of the  scaffold hanging siding. It was clearly visible that the "pik" was not level. A competent person did not inspect this scaffold prior to use to detect this hazardous condition.    NO ABATEMENT CERTIFICATION REQUIRED
