29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 19, 2012, four employees were attaching the walls to the metal building under construction without the use of hard hats, exposing employees to the hazard of being struck by tools or materials.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about November 19, 2012, four employees were attaching the walls and overhead to the metal building under construction using portable drills without eye protection, exposing employees to the hazard of eye injury.

29 CFR 1910.178(l)(1)(i):  The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about November 19, 2012, an employee was operating a JLG all-terrain forklift without the employer ensuring that the operator was competent and trained to operate the powered industrial truck, exposing the employees to a fall hazard.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about November 19, 2012, an employee was attaching the walls to the metal building under construction while elevated up to approximately 14 feet high while standing on a wooden pallet that was on the forks of the JLG all-terrain forklift without fall protection, exposing the employee to a fall hazard.
