29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) BCS Construction, LLC on or about November 19, 2012, at the address of 2115 W. Michigan St. Sidney, OH: The employer failed to ensure the scaffolding level from which employees were installing exterior sheeting was fully planked. Two 19 in. planks were missing from the 5 ft. wide scaffolding frame, exposing the employees to a 14 ft. fall to the ground below.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. a) BCS Construction, LLC on or about November 19, 2012, at the address of 2115 W. Michigan St. Sidney, OH: The employer did not ensure employees were protected from a fall while working from a supported scaffold. The employees installing exterior sheeting from the scaffolding platform without standard railing were exposed to a 14 ft. fall to the ground below.
