Michigan St. Sidney, OH:  The employer failed to ensure the scaffolding level from which employees were installing exterior sheeting was fully planked.  Two 19 in. planks were missing from the 5 ft. wide scaffolding frame, exposing the employees to a 14 ft. fall to the ground below.
