29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    Employees re-roofing a single story house were not using proper fall protection:    a) Two employees had rope lifelines tied around their waists.    b) One employee had a snap hook connected to his chest strap.    c) A ridge anchor connector was secured to the roof with two roofing nails on each side of the roof.    d) A rope lifeline was tied to the roof anchor with one employee on each end of the rope.    5 Star Roofing was previously cited for a violation of this Occupatiional Safety and Health Standard, or an equivalent Standard (1926.501)(b)(11), which was contained in OSHA Inspection Number 315466029, Citation #1, Item #3a, whcih was affirmed as a Final Order on  7/8/11, with respect to a workplace located at N6051 Hwy P, Hellenville, WI 53137.    5 Star Roofing was previously cited for a violation of this Occupational Safety and Health Standard, or an equivalent Standard (1926.501)(b)(11), which was contained in OSHA Inspection Number 310765862, Citation #1, Item #2, which was affirmed as a Final Order on 6/10/08, with respect to a workplace located at 2574 Honey Creek Cir., East Troy, WI 53120.
