29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid point: a) 2471 6th St., Fort Lee, NJ: Employee engaged in roofing work, was acessing the roof surface by means of a ladder that did not extend 3 feet above the landing nor was it secured, exposing employees to a fall of approxiamtely 12 feet to a lower roof, on or about 11/19/2012.
