On or about 11/16/2012, Omega Engineering, LLC - Walgreens- St Thomas, Lot 5 & 5A, St Thomas V.I. Concrete slab foundation area: Employees were exposed to a fire hazards were working installing PVC conduits using a fire torch attached to a LPG tank which its pressure gauge regulator was damage. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

Concrete slab foundation area: Employees were walking nearby unprotected steel rebars while accessing the construction site. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
