Two (2) floor holes measuring approximately 2 feet by 4 feet and 3 feet by 4 feet and 4 inches deep were in the general walkways.

29 CFR 1926.502(b)(4): When the 200 pound (890 N) test load specified in paragraph (b)(3) of this section was applied in a downward direction, the top edge of the guardrail deflected to a height less than 39 inches (1.0 m) above the walking/working level: Jobsite: 12th, 11th, 10th floors. a) Employees were exposed to falls where the perimeter guard ropes were resting at 39 inches above the walking/working surface and deflected, in some cases, down to 35 inches. Jobsite: 5th floor. b) The wire rope guard rope around the duct shaft was loose and sagging at 37 inches from the walking/working surface.
