29 CFR 1926.403(b)(i):  Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification for that identified purpose:    (a) Miramar Construction, Inc., @ 2015 Market Street, Denver, CO.  On or before November 21, 2012, the exposing employer did not ensure that 120- volt relocatable power strip, which was being used in the construction site to power portable drills, was used in accordance with the instructions included listing or labeling, or certification of that identified equipment purpose and location.  The condition exposed employees to an electrical hazard.    Note: Relocatable power taps are intended to be directly connected to a permanently installed branch circuit receptacle. Relocatable power taps are not intended to be series connected (daisy chained) to other relocatable power taps or to extension cords. Relocatable power taps are not intended to use at construction sites and similar locations. Relocatable power taps are not intended to be permanently secure building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring. The cords of relocatable power taps not intended to be routed through walls, windows, ceiling, floors or similar openings Relocatable power taps are intended for indoor use to power laboratory equipment, home workshops, home movie lighting controls, musical instrumentation, and to provide outlet receptacles for computers, audio and video equipment, and other similar equipment.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal arrest system:    (a) Miramar Construction. On or before November 21, 2012, the exposing employer did not ensure that fall protection systems were utilized to safeguard employees from falling while employees performed work on the commercial building under construction in the following instances:    1.Unguarded/Uncovered floor opening on the 3rd level being sheathed     2.Unguarded floor opening, employees ladder point of access to the 3rd level.  3.Unprotected side and edge, employees point of access to the lower level of the building.    These conditions exposed employees to the hazard of falling from elevation up to a height of 8 feet to lower level.

29 CFR 1926.701(b):  All protruding reinforcing steel, onto and/or into, which employees could fall or come against, shall be guarded to eliminate the hazard of impalement:    (a) Miramar Construction, Inc., @ 2015 Market Street, Denver, CO.  On or before November 21, 2012, the exposing employer did not ensure each protruding reinforcing re-bar, which was sticking out 28 above the floor at the elevator shaft, 3rd level, was capped or guarded to eliminate the hazard of impalement.   This condition exposed employees to an impalement hazard.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Miramar Construction, Inc., @ 2015 Market Street, Denver, CO.  On or before November 21, 2012, the exposing employer did not ensure that the ladder used by the employees to gain access to the 3rd floor was extended at least 3 feet above the floor surface for safe access. This condition exposed employees to the hazard of falling from elevation at a measured distance of 66 to the lower level.
