29 CFR 1910.184(h)(5): Removal from service. Natural and synthetic fiber rope slings shall be immediately removed from service if any of the following conditions are present such as broken or cut fibers:      a) On or about  November 16, 2012, in the Loading Department, the employees were using a 2 foot wide Pro Edge synthetic fiber sling, that was severely cut and frayed, exposing employees to the hazard of being struck by the load.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.      a)On or about November 16, 2012, the Kidde fire extiniguisher was not mounted and identified, exposing employees to the hazard of fire.
