29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Building 42 Roof, Springfield Apartments - On or about 23 November, 2012 - Employees were removing roofing shingles on a 5 on 12 pitch roof over 6 feet above the lower level without the use of fall protection.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Building 42 Roof, Springfield Apartments - On or about 23 November, 2012 - Employees were cleaning the ground area and loading debris into the trailer, without the use of hardhats while employees were working on the roof directly 24 feet above them.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    a) Building 42 Roof, Springfield Apartments - On or about 23 November, 2012 - Employees were using an extension cord to power a leaf blower on the roof that was not protected by a gorund fault circuit interruptor.
